Patient Centered Medical Home

The Patient Centered Medical Home (PCMH) is a nationally-recognized primary health care model for children, adolescents and adults. The PCMH improves teamwork, trust and partnerships between patients, personal physicians, mental health specialists, and, when appropriate, the patient’s family.

Three of CHP’s primary care practices — CHP Neighborhood Health Center, CHP Lee Family Practice and CHP Great Barrington Community Health Center — have earned designation as Patient Centered Medical Homes from the National Committee for Quality Assurance (NCQA).

CHP fully supports the PCMH model and assists all of its practices in improving the quality, effectiveness and efficiency of care they provide to patients, and ensures that health care practitioners focus first on meeting the needs of patients. Research shows that the PCMH approach leads to higher quality and lower costs, and can improve patient and Clinician health care experiences.
